Why we need medical release forms

Medical release forms from all convention participants under the age of 18 are required. These forms are important to ensure the safety of participants. These forms are used in emergency situations where medical assistance is needed. They also alert convention planners to any current health issues or special needs, particularly with children.

Note: sponsors should keep a copy of all forms for their youth group with them at all times.
